Despite what you've been told, a disc herniation doesn't automatically mean you need surgery or that your days in the gym are over. With the right recovery strategy, many people return to lifting, running, sports, and everyday life stronger than before.
The key is knowing what to do first and when to progress.
In this guide, you'll learn the same two-phase recovery approach we use to help people reduce pain, restore movement, and safely return to heavy lifting.
Why Do Disc Herniations Happen?
One of the biggest misconceptions is that a disc herniation happens because you simply "lifted something wrong."

In reality, most disc herniations develop over time.
Poor movement habits, repetitive bending, stress, lack of recovery, poor sleep, inactivity, and excessive training load gradually increase stress on the spine.
Eventually, one movement- whether it's a heavy deadlift or simply picking up your shoes- becomes the final straw.
The most commonly injured disc is the L5-S1 disc, located at the bottom of the lumbar spine.
This disc absorbs tremendous amounts of force every day because it supports much of your body weight while allowing you to bend, lift, twist, and move.
When excessive stress exceeds what the disc can tolerate, part of the disc can bulge or herniate backward toward nearby nerves.
This often causes symptoms such as:
Low back pain
Sciatica
Pain while bending forward
Difficulty standing upright
Stiffness
Muscle guarding
Loss of mobility
Many people also notice they wake up the morning after the injury feeling "locked" into a bent-forward position.
These are classic signs of a lumbar disc herniation.
Do You Need an MRI?
Many people panic after hearing the words disc bulge or disc herniation on an MRI.

However, research consistently shows that imaging doesn't always match symptoms.
In fact, many people with no back pain at all have disc bulges visible on MRI scans.
That's why healthcare professionals don't rely solely on imaging.
Instead, they evaluate:
Your symptoms
Your movement
Your strength
Your mobility
Which movements improve your pain
Which movements make symptoms worse
Your body, not your MRI, is often the best guide for recovery.
The encouraging news is that the vast majority of lumbar disc herniations improve without surgery when managed appropriately.
Understanding the Two-Phase Recovery Method
Returning to the gym isn't about jumping back into heavy squats as soon as the pain decreases.
Recovery should happen in two stages.
Phase 1: Reduce
The goal is to:
Calm the irritated tissues
Restore spinal movement
Reduce pressure on the nerve
Improve mobility
Reduce pain
Only after these goals are achieved should you begin rebuilding strength.
Phase 1: Reduce the Disc Herniation
Exercise 1: 90/90 Belly Breathing
How to Perform 90/90 Belly Breathing
Lie on your back.
Place your calves on a chair or couch.
Keep both hips and knees bent to approximately 90 degrees.
Place one hand on your stomach and one on your chest.
Take slow breaths into your belly.
Allow your abdomen to expand in all directions.
Exhale slowly while keeping your shoulders relaxed.
Perform for approximately 5 minutes once or twice daily.
Why It Helps
Calms the nervous system
Reduces pain sensitivity
Relaxes protective low back muscle guarding
Improves spinal relaxation before mobility work
Creates a better environment for healing
Many people skip this step, but reducing muscle guarding often makes the following exercises significantly more effective.
Exercise 2: Prone on Elbows
How to Perform
Lie on your stomach.
Prop yourself onto your elbows.
Relax your lower back and glutes.
Stay in this position comfortably.
Slowly move in and out of extension if tolerated.
As this becomes easier, progress to pushing up onto your hands.

Restores extension mobility
Reduces pressure on irritated nerves
Improves spinal movement
Encourages disc remodeling
Helps reduce sciatica symptoms
One encouraging sign is if leg pain begins moving upward toward the back instead of traveling farther down the leg.
Exercise 3: Core Bracing Leg Lifts
Mobility improvements don't last unless your body learns to control them.
That's why stability is the next step.
How to Perform
Lie on your back.
Bend both knees.
Gently flatten your lower back.
Brace your core.
Slowly lift one leg at a time.
Alternate for approximately 20 repetitions per side.
Why It Helps
Improves spinal stability
Activates deep abdominal muscles
Maintains mobility gains
Builds confidence with movement
Reduces unnecessary stress on the injured disc
This completes the Reduce Phase.
Some individuals remain in this phase for two weeks, while others require longer depending on symptom severity and recovery progress.
Phase 2: Rebuild the Spine
Once pain decreases and movement improves, it's time to rebuild your body's capacity.
This phase prepares you for everyday activities first and eventually the gym.
The goal is no longer simply reducing pain.
Now you're teaching the spine to tolerate load again.
Exercise 1: Bear Hold
How to Perform
Begin on hands and knees.
Lift both knees slightly off the floor.
Maintain a neutral spine.
Brace your core.
Hold for 20–60 seconds.
Why It Helps
Builds full-core stability
Improves trunk endurance
Protects the lumbar spine
Develops better control before lifting
Exercise 2: Kettlebell Dead Bug
Once the basic core exercise becomes easy, it's time to increase the challenge.
How to Perform
Lie on your back.
Hold a kettlebell or dumbbell above your chest.
Brace your core.
Lower one heel toward the floor.
Return to the starting position.
Alternate sides.
Aim for approximately 20 repetitions per side.
Why It Helps
Improves anti-extension strength
Builds deep core control
Increases intra-abdominal pressure
Reinforces spinal stability before heavier lifting
Returning to Squats
One of the biggest mistakes people make is jumping directly back to heavy barbell squats.
Instead, begin with the Goblet Squat.
Holding the weight close to your chest naturally encourages a more upright posture, reducing stress on the lower back while allowing you to rebuild leg strength.
Benefits of Goblet Squats
Upright torso
Lower spinal loading
Rebuilds squat mechanics
Excellent transition back to training
Gradually increase:
Weight
Depth
Confidence
Only progress if symptoms remain controlled.
Returning to Deadlifts
Instead of immediately returning to a conventional barbell deadlift, begin with the Trap Bar Deadlift.
The trap bar allows a more upright body position while reducing stress on the lumbar spine.
As strength improves, progress to a controlled hip hinge using a wall as feedback.
Wall Hinge Progression
Stand several inches away from a wall.
Push your hips backward until they gently touch the wall.
This teaches proper hip hinging while limiting excessive forward bending.
As tolerance improves:
Move farther from the wall.
Increase hinge depth.
Add resistance gradually.
Eventually, you'll be ready to return to conventional deadlifting.
The key isn't rushing.
The key is progressing safely.
Why Random Gym Exercises Won't Get You Back to Lifting
One of the biggest mistakes people make after a disc herniation is searching for random "herniated disc workouts" or jumping back into gym exercises they find online.
The reality is that getting back to the gym after a disc herniation isn't about doing more exercises—it's about doing the right exercises at the right stage of recovery.
A successful return to lifting depends on:
Choosing the right exercises for your specific symptoms
Knowing when it's safe to progress your training
Monitoring how your body responds to each movement
Gradually rebuilding your tolerance to load
That's why the best recovery plans are individualized. What works for someone else in the gym may not be the right starting point for you.
The Goal Is to Prepare Your Spine for the Gym Again
One of the most important concepts in disc herniation recovery is centralization.
As your spine begins to recover, your symptoms should gradually move:
OUT of the leg
BACK toward the lower back
This is often a positive sign that your body is responding well and that you're moving closer to safely returning to lifting.
Before you think about heavy squats, deadlifts, or other gym exercises, your spine needs to demonstrate that it can tolerate movement without increasing nerve irritation.
What Your Spine Needs Before You Return to the Gym
Getting back to the gym after a disc herniation isn't about forcing your way through pain or avoiding exercise forever.
It's about rebuilding your body's capacity to handle load again.
That means improving the function of your:
Intervertebral discs
Nerve roots
Lumbar spine
Core stability
Movement patterns
When mobility, stability, and strength are rebuilt in the correct order, you'll be in a much better position to safely return to squats, deadlifts, running, and the activities you enjoy.
